Please note: The following requests can take up to 31 working days to be complete. There is a charge for these services which you can see below. This charge will need to be paid in full by cash or card before your request can be processed.

Fee's and Charges for Patient Information Sheet

Private Patients
If you need to see a clinician privately, charges are as follows:
GP - £60
Nurse - £20
Private Prescription - £15 (Please not a charge of £8.60 will also be charged at the pharmacy)

Travel Vaccinations
If you require travel vaccinations before departing on holiday, you will need to complete a travel vaccination form which you can request from reception or under the clinic and services tab. This form will need to be completed 6 weeks before the date of travel. Certain vaccinations carry a charge. Please see Fees and Charges before booking an appointment.

Medicals
Our GP's are happy to complete a medical examinations for patients who require one for accident/sickness/insurance and pre-employment. We also complete  driver/licencing and taxi medical examinations. 

Insurance
Any insurance documents i.e. holiday cancellation forms, reports, medical examinations or copies of medical notes can also be completed by us.

Letters of Fitness
If you require a letter of fitness to fly or exercise etc, you can request this by putting what you require in writing.

Access to Medical Records

You are entitled by law to have access to your medical records. 

If you would like to see your medical records or you would like a copy of your medical records, you will need to put this request in writing stating what information you need i.e. between specific dates or about a particular condition.

Please note:
If you are registered for SystmOnline, you can request to view your medical records online. This may take the surgery 72 hours before approving your request.
