Self Certification Sick Note for less than 7 days
If you have been ill for 7 days or less you don’t need to see a Doctor. You can complete a Self Certification form yourself. However some employers insist on a Doctor’s note regardless. A Doctors note under these circumstances involves a charge of £40.Many employers have their own self-certification forms. If your employer doesn’t have its own form your can download this Self Certification Form. Please print it, fill it in and hand it in to your employer. You do not need to see a Doctor.
Self Referral Counselling - for young people aged 10 - 25 years
Sorted offers a free and confidential self referral service for young people aged 10 and 25 years of age, usually based at our Grove Corner offices, with our own discreet entrance and counselling rooms.
Our phones are manned by experienced counsellors who will listen sympathetically and help you decide if you would like to attend an initial appointment. We will try our best to find times that suit you best.
Sometimes we do have a short waiting list, but we are here to support you if you have to wait a while for your first appointment.
Counselling sessions are offered between 10 .00 a.m. & 6.30 p.m.
Monday - Friday, dependent on spaces available
No problem is too large, or too small for you to share it with us

Self Referral - Bedfordshire Well Being Service
The Bedfordshire Wellbeing Service is for residents of Bedfordshire who experience depression, anxiety, sadness, extreme shyness, obsessive behaviour, phobias, relationship difficulties including depression and common mental health symptoms or other psychological issues which are holding them back in their lives. It provides one-to-one sessions and/or group workshops. You can self refer yourself via email: Elt-tr.bedfordIAPT@nhs.net
Self Referral - Audiology and Optometrist
Children under 16 can contact Liverpool Road Health Centre and book in for an audiology or optometrist appointment. Please call 01582 708100
Self Referral - District Nurses
Community nursing services include district nurses and community matrons. District nursing teams assess, plan and provide nursing clinical care to those people who are often housebound due to ill health, either in their own home, or in a care home that does not provide nursing.
They also support:
- End of life care
- post operative care
- management of long term conditions
- wound care.
Who can use the service?
Anyone aged 18 or over registered with a Luton GP and who is unable to leave their house due to physical or medical condition.

Press 1 - If you're a patient.
Press 2 - For staff in GP surgeries.
Press 3 - For East of England ambulance or paramedic personnel.
Referrals for behavioural problems/anger issues/Possible ADHD/ADD
Please note the school can refer directly to the Edwin Lobo Centre, the school SENCO team can arrange a referral, the patient does not need to see a GP for this referral to be made.
