The following requests can take up to 31 working days to be complete. There is a charge for these services, which you can see below. This charge will need to be paid in full, by cash or card before your request can be processed.

Travel vaccinations

If you require travel vaccinations before departing on holiday, you will need to complete a travel vaccination form which you can request from reception.

This form will need to be completed 6 weeks before the date of travel. Certain vaccinations carry a charge.

Medicals

Our GPs are happy to complete a medical examinations for patients who require one for accident/sickness/insurance and pre-employment.We also complete driver/licencing and taxi medical examinations.

Insurance

Any insurance documents, such as holiday cancellation forms, reports, medical examinations or copies of medical notes can also be completed by us.

Letters of fitness

If you require a letter of fitness to fly or exercise etc, you can request this by putting what you require in writing.

Access to medical records

You are entitled by law to have access to your medical records.

If you would like to see your medical records or you would like a copy of your medical records, you will need to put this request in writing stating what information you need i.e. between specific dates or about a particular condition.

If you are registered for SystmOnline, you can request to view your medical records online. This may take the surgery 72 hours before approving your request.
